JAT Rx Offers Three Easy Ways to Place an Order

1) NEW! Online ordering. Simply log in and select the Shop JAT Rx tab at the top of the web page.

2) Order by fax-Log in. From the top menu hover over Pharmacy Services. Select JAT Rx Order Form. Fill out form, sign and fax.

3) Order by e-mail-Log in. From the top menu, hover over Pharmacy Services. Select JAT Rx Order Form. Fill out form, create a digital signature and e-mail.

JAT Pharmacy, LLC, is currently unable to ship to: Alaska, Arizona, California, Hawaii, Montana, North Carolina and Oregon.
